JOB SPECIFICATIONS:
LicensedRegistered Nurse in the state of LA preferred
Acceptabledegree in health sciences such as Nutrition pharmacy, physical therapy, mentalhealth therapists, personal trainers or licensed practical nurses
Graduateof a State approved health program such as nursing or allied health professionsrequiring licensing.
Preferredexperience in an ambulatory setting with adults, focused on primary care andmanagement of patients plans of care with a focus on living with a chronicdisease.
JOB SUMMARY:
AHealth Coach assesses client's lifestyles, addresses health concerns and worksdirectly with patients to discuss goals, progress actions and challenges facingthem that influence their ability to improve their wellness and overall healthstatus. The partnership role providesa bridge between primary care provider visits, facilitates change in healthlife-style habits while supporting patients in their learning journey to managetheir self-care actions. Assisting patients to recognize their personal values,internal strengths and insights to mobilize those assets thereby helping themreach their life-style goalsandcollaborative plan of care with the Provider.Through a growth promoting partnership with patients the healthcoach assists clients to achieve these changes that can be rewarding and sustainable.Utilizes clinical knowledge to improve the patients' health literacy andknowledge of their chronic disease process. Educates patients on clinicalmeasures specific to their health status and medications that impact theirchronic disease.
Specific Duties andResponsibilities:
  • Contacts and performs initial interviews with individuals who are eligible for the Health Coaching program.
  • Conducts regular meetings in person, virtually or through conference phone calls to discuss self- management goals, concerns, challenges and barriers to achievement.
  • Provides guidance to patients through the process of creating a shared vision for their health and well-being, developing healthy habits such as diet management, exercise regimen, and compliance with plan of care as mutually developed with primary care Team(provider, Health Coach and clinical pharmacist)
  • Collaborates with the Care Team to include the clinical pharmacist Health Coach in the medication therapy management plans of care.
  • Provides direct assistance to patients with their current physical and emotional state through goal setting with establishment of self-management goals and works with them on plans of action to move in a positive direction in achieving these goals.
  • Assists patients in understanding the interconnectivity of relationships, community, home, finances and emotional health in dealing with their chronic disease and achieving an improved lifestyle and reducing or eliminating behaviors that are considered high risk.
  • Assists the patient in the understanding of the treatment plan, diagnostic tests, therapies suggested by the provider in their individualized plan of care.
  • Through effective coaching assists patients to achieve improved specific clinical measures that are monitored regularly by the Care Team.
  • Provides direct care management improving follow-up on referrals to Provider sub-specialists linking their care goals with the primary care Provider Team
  • Serves as a translator of health information from the primary care provider and Pharmacist drug therapy Coach to the patient in an effective and supportive approach.
  • Coordinates referral assistance where appropriate and provides an open line of communication between provider visits.
  • Solicits patient feedback through surveys and assessments aimed at improving coaching services to increase satisfaction with the Healthy Patient Healthy Life Intersect program.
  • Works collaboratively with other disciplines (external and internal) that the patient engages with to support a multidisciplinary approach to care management.
  • Monitors patient scheduling processes to achieve compliance with appointments.
  • Monitors clinical care indicators established with the patient providing them with a report card of performance and regular discussion of methods for improvement in a supportive counseling style.

• Honors that each patient is an expert in his orher life while ensuring that all interactions are respectful andnon-judgmental.
Required knowledge, skillsand abilities:
  • Exhibits highly effective communication skills for working with patients, their families, and other health care providers.
  • Utilizes their knowledge of health-related chronic diseases and illness and typical medical interventions used to address them.
  • Demonstrates a commitment to patients and a willingness to go above and beyond normal responsibilities to provide care management in a supportive manner.
  • Demonstrates innovation, acceptance of new ideas and concepts in order to display above average optimism and the ability to motivate patients to make vital changes in their lives.
  • Demonstrates the ability to resolve conflicts with patients and overcome their objections about their health and behaviors.
  • Displays self-confidence in performing the skills of communication, empathy, and mindfulness using listening techniques and motivational interviewing to gain insight into the patient's perspective on their chronic disease.
  • Assist patients in taping into their own existing capabilities to create a shift in their own mindset of a belief of a healthier life that is realistic.
  • Possess a desire to continue learning about health, wellness, and best coaching practices.
  • Achievement of a certification in Health Coaching through the identified course selected by the organization.
  • Documents efficiently in the patient's record the Health Coaching and other actions associated with the plan of care.

• Participates in performance improvementactivities as evidenced by the development of measures, collection of data,analysis of data, and attendance at Quality Meetings.
Qualifications:
Significant knowledge of medical practices andprocedures in a primary care environment.
Abilityto relate with effectiveness to patients, families, other providers, communityleaders and clinic staff.
Abilityto function effectively as a Team Member.
Physical Requirements:
• The physical demands described here are representative of those thatmust be met by an employee to successfully perform the essential functions ofthis job. While performing the duties ofthis job, the employee is regularly required to walk, sit, talk and hear and beable to operate a personal computer, printer, telephone, calculator, andphotocopier and fax machine. Must have excellent hearing and verbal skills tointeract with the public. In addition, must have abilities to communicate inwriting and orally. Fluency in written and spoken English is required.
• Must have coordination of sight and body movements for variousapplications of procedures and dexterity of finger and wrist to operate thecomputer for documentation
• Exposure to various chemicals agents typically used in an office clinicsetting
• Walking, bending, and occasional lifting of 40 - 50 lbs. andpushing of 5-20 lbs.
• Ability to use the computer in basic word processing, excel software,and EMR with training.
• Job responsibilities may require, walking,sitting, lifting, and travel in own vehicle for company business.
