This is a full-time remote position.
The hours for this position are 8a-5p in time zone of residence.
Position Summary
Provides subject matter expertise, consultations, case reviews and education to clinical staff and members. • Participates in the inter-disciplinary team and collaborates with healthcare team to align nutritional interventions with care goals. • Evaluates the plan of care and effectiveness/outcomes of dietary interventions and modifies as needed. • Educates members and families on healthy diet, appropriate nutrition guidelines for disease state(s), provides prevention and wellness, guidance. • Connects members with nutritional community resources, dietary counseling services, leveraging applicable nutritional plan benefits. • Collaborates with nutritional therapy providers, home care providers , community resources for nutritional. • Utilizes evidence based knowledge and protocols appropriate to patient populations and diagnoses. • Adheres to care management and quality management processes in compliance with regulatory and accreditation guidelines and company policies and procedures. • Other responsibilities as assigned.
Required Qualifications
3+ years related work experience as a Registered Dietitian
Must possess or be willing to obtain Registered Dietitian licensure in the following states: FL, PA, TX, IL, NC, NJ, OH, NY, GA
Must be bilingual English / Spanish
Preferred Qualifications
Dietitian licensure in ALL states: FL, PA, TX, IL, NC, NJ, OH, NY, GA
Education
Must have a BA or MA in nutrition
Pay Range
The typical pay range for this role is:
$46,597.20 - $101,200.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
