Job Summery:
Provide, maintain, and restore participant's health by coordinating the healthcare process; completing assessments, collaborating with physicians and staff to provide physical and psychological support, and provide medication management. Coordinate strong support from the caregiver, family members ,and staff. Accurate documentation and progress notes on each participants.
Principle Duties & Responsibilities :
- Coordinate medication administration per authorization
- Communicate and coordinated with participant's physician to maintain current medications, refills and authorization of over the counter medication.
- Contact participant's physicians regarding adverse reaction to any medication .
- Monitor and document any and all unusual behavior, change in mood, change in attitude, suggestion of family problems or personal problems or need for help or community services.
- Record all medications being brought into the facility
- Ensure the proper procedures are executed concerning MAR documentation and medication disposal policies are followed.
- Identifies participants care requirements by establishing personal rapport with potential and actual participants and other persons in a position to understand care requirements .
- Establish a compassionate environment by providing emotional ,psychological and spiritual support to patients, friends ,and family
- Promote participant's independence by establishing participants care goals; teaching patient, friends and family to understand condition, medication and self- care skills; answering questions .
- Assures quality of care by adhering to therapeutic standards; measuring health outcomes against participants care goals and standards; making or recommending necessary adjustments; following hospital and nursing division's philosophies and standards of care set by the state board of nursing ,state nurse practice act, and other governing agency regulations .
- Maintain safe and clean working environment by complying with procedures, rules and regulations; calling in for assistance from the Health Care support personnel.
- Protect patients and employee by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations.
- Documents participants care services by charting in participants and department records.
- Maintains participants confidence and protects operations by keeping information confidential.
- Ensures operations of equipment by completing preventive maintenance requirements; following manufacture's instructions; troubleshooting malfunctions; calling for repairs; maintaining equipment inventories ;evaluating new equipment and techniques.
- Maintains nursing supplies inventory by checking stock to determine inventory level; anticipating needed supplies; placing and expediting orders for supplies ;verifying receipt of supplies ;using equipment and supplies needed to accomplish job results.
- Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks ;participating in professional societies.
- Maintains a cooperative relationship among Health Care teams by communicating information; responding to requests; building rapport; participating in team problem-solving methods.
- Contributes to team efforts by accomplishing goals.
- Assist Registered Nurse in completing Initial l Assessments ,quarterly assessments and six-month assessments.
- Ensure annual exams are completed for each participant by their person centered plan on file .
- Qualifications and Requirements

- The Health Coordinator - Registered Nurse (RN) currently licensed in NC with a minimum of 3 years licensed nursing experience. Associates degree required ;BS degree preferred. Shall have a knowledge and understanding of the physical and emotional aspects of aging and resultant disease and infirmities and related medications and rehabilitative measures. Shall have a signed medical medical statement by a physician within the last 12 month prior to employment
