We offer a continuum of programs addressing substance abuse, mental health, adolescent issues, family relationships, and more. Every year, H.A.S. serves over six thousand individuals.

Essential Functions:

Under the direct supervision of the Program Manager, this position is responsible for offering technical assistance and case management linkage and referrals as needed. Additionally, the Case Manager coordinates with the treatment team the plan of care for participants with mental illness and/or substance use issues. A Harm Reduction Case Manager acts as a liaison between participant/family and clinician to ensure that their non-clinical case management needs are being met.

Harm Reduction Case Manager Responsibilities:

  • Manages a caseload of participants to ensure adequate services are being provided to assist participants with meeting their treatment needs
  • Participates in regular staff meetings held at position location.
  • Links participants to needed and available community resources including legal,medical, housing, transportation, mental health, financial, etc.
  • Provides ongoing supportive and/or case management functions in accordance with the problems, needs, and the strategies identified within the service/treatment plan in order to help the participants to achieve the stated goals and objectives.
  • Provides face to face reviews with the participant and/or relevant staff on a regular basis. Documents all service contacts on a timely basis including face-to-face interviews, collateral and networking contacts, correspondence and maintains the case records in accordance with agency and regulatory standards and requirements.
  • Collaborate with the treatment team to ensure compliance and to work toward improved participant outcomes.
  • Provides anti-stigma education to the community and individuals dealing with mental illness and/or substance use disorders as well as information on prevention, treatment, and recovery for participants.
  • Meets regularly with the Program Manager as a means of enhancing professional growth, reviewing and processing the provision of case management services, and dealing with appropriate administrative issues.
  • Maintains a networking relationship with community agencies, clinics and supportive services for referrals/linkages and ancillary services. Provides specific case management support, services, and documentation associated with any private or public grants.
  • Provides monthly progress reports to Program Manager.
  • Performs other duties as assigned by the Program Manager.

Harm Reduction Case ManagerQualifications:

  • Work will require traveling and will require the candidate to be onsite.
  • Strong organizational, communication, and critical thinking skills, including the ability to work as part of a collaborative treatment team.
  • Basic knowledge of computer keyboard.
  • Bilingual/Bicultural is preferred (English/Spanish).
  • Must be flexible with hours to accommodate participant needs.
  • Possession of valid Illinois Driver’s license and willingness to use personal vehicle in course of employment.

Education and Experience Requirements:

  • Bachelor’s degree in a human services field, like social work, psychology or sociology or from a regionally accredited educational institution which has U.S. Department of Education approval, is required; or a minimum of two years in college working on degree is preferred.
  • Have two years of case management experience in a non-profit setting preferred.
