PRIMARY FUNCTION:

Provide oversight and management of the DSI related Innovation Pilot Program (IPP) grants. The IPP grants vary with topics tied to health and wellness and employment services. This role will ensure grants are implemented fully throughout all regions of DSI. This position will end March 2025.

ESSENTIAL FUNCTIONS:

  • Manage and organize the implementation of the IPP grants across all regions of DSI.
  • Meet with participants to collect all necessary paperwork. This includes, but is not limited to consent forms, quarterly surveys, completing quarterly reports, and completing intake paperwork for the Business Champion IPP grant.
  • Host quarterly town hall meetings for participants, potential participants, guardians, and case managers.
  • Serve as a liaison between participants and the Employment Services Department for the Business Champion IPP grant,.
  • Serve as a liaison between participants and Nursing Services for all health and wellness related IPP grant involvement.
  • Collect quarterly invoices from IPP collaborating partners.
  • Review data collected to create and submit quarterly billing invoices to BDS.
  • Act as the point of contact for any interested participants and create a wait list as needed. Manage the Formstack signup form.
  • Share marketing materials with community stakeholders and host informational sessions for the Business Champion IPP.
  • Train the DSI employees who are involved with the IPP grants, on how to carry out the expectations, intent, and purpose of these projects.
  • Audit the case notes and reports of the employees working with the project participants.
  • Provide weekly updates and reports to the Chief Program Officer.
  • Schedule and host quarterly IPP collaborator meetings.
  • Ensure the programs are operating in compliance with the grant standards.
  • Assist in job site visits with the Employment Specialists and Business Champions.
  • Develop an electronic participant survey, analyze data, and create a monthly report.
  • Present updates and data to the senior leadership team, board members, and community as needed.
  • Work with the DSI nurses and participants in the health and wellness project to gather data and maintain data tracking.
  • Strictly maintain confidentiality of all project participants.
  • Be willing to be flexible in work hours, including some evening and weekend hours as needed.
  • Prioritize multiple job functions, coordinate routine responsibilities with spontaneous work assignments.
  • Work alongside the Developmental Department as needed for special events.
  • Work alongside the Nursing Department to coordinate an agency health fair.
  • Work alongside the Employment Services Department to coordinate an employment resource fair for the Business Champion IPP grant.

OTHER FUNCTIONS:

  • Promote agency philosophies, generate team spirit, create and maintain an environment that fosters employee satisfaction, maximizes productivity and profitability.
  • Assist with planning and participation of agency events, as needed.
  • Attend, plan, and schedule IPP grant collaboration meetings.
  • Be available to travel to all regions of DSI throughout the state.

QUALIFICATIONS:

  • Bachelor’s degree or equivalent experience in a human service field preferred, plus at least one year grant management experience.
  • Must possess excellent communication skills both written and oral, and strong teamwork skills.
  • Must have the ability to successfully work in both independent and group settings. Must be capable of communicating professionally and effectively.
  • Must be able to work a flexible schedule, including some evenings and/or weekends, to meet the needs of regional staff.
  • Maintain a valid Indiana driver’s license, insurance on personal vehicle (minimum of PLPD with Medical liability coverage), and maintain insurability as defined by the agency liability insurance carrier.
  • Must possess and maintain reliable transportation that meets state requirements for transporting clients.
  • Possess and maintain a clear criminal record as required by Medicaid Waiver regulations.
  • Must possess or be able to obtain CPR and first aid certification annually and perform as necessary.
  • Must possess or be able to obtain NCI certification annually and perform as necessary.
  • Must be able to perform such functions as operate a computer, access existing records storage, attend meetings throughout the community, and lift up to 25 pounds.
