Hudson Guild is seeking a passionate and driven Case Manager for our Geriatric Mental Health Initiative Program (GMHI). We are seeking an individual who is organized, caring and has respect for diversity, including culture, ethnicity, and sexual orientation.


The Geriatric Mental Health Initiative (GMHI) Program is a grant funded initiative designed to identify unmet mental health needs of the older adults within our community with the goal of connecting them to mental health services. Operating under Hudson Guild’s Mental Health Services team, the GMHI program works with each of the Older Adult Services teams within the agency to screen older adult participants for mental health concerns, and to provide referrals to individual and group therapy services both within the agency and within outside community clinics.


POSITION OVERVIEW:

Under the supervision of the GMHI Program Coordinator, the Case Manager will be responsible for providing administrative and program development support to the GMHI program, case management and informal supportive counseling services to GMHI program participants, and will assist the Mental Health Services team with patient care coordination. The Case Manager must possess a strong working knowledge of government entitlements and social service systems.


REQUIRED QUALIFICATIONS: Bachelor’s degree in human services, Social Work or other related field; 1 to 2 years’ experience in the health and human service field, including experience with informal counseling/ providing direct emotional support to clients, the identification of/knowledge of community resources and benefits programs and case management skills. Excellent organizational, communication, outreach, and advocacy skills and the ability to work as a team. Flexibility with schedule; it may be necessary to work some evenings/Saturdays. He/She/They must possess excellent time management, attention to detail, organizational and communication skills and maintain the integrity of the organization at all times.


PREFERRED QUALIFICATIONS: Two years of related work experience preferred. Must be able to speak, read and write in English; Spanish or Cantonese is preferred. BA/BSW is required.


RESPONSIBILITIES:


  • Conduct GMHI screenings and needs assessments as assigned using program specific tools
  • Develop and facilitate emotional health/wellness support groups as assigned by supervisor
  • Provide informal supportive counseling to a caseload of up to 8 older adult clients holding 30-minute, weekly in-person meetings/phone calls
  • Collaborate with program directors of Adult Services and Supportive Housing programs to develop community events that promote GMHI services
  • Provide services ranging from concrete services (benefits and entitlements), to case assistance and case management, group programming, advocacy, resources and referrals
  • Assist participants with case management and assistance needs
  • Provide information/ referrals/ linkages to other appropriate services
  • Maintain, and complete timely and appropriate documentation in designated EMR system including progress notes, case contacts, case summaries, and produce monthly reports
  • Comply with all funder service standards and contract requirements
  • Participate in case presentations, staff meetings, field visits and regular training
  • Participate in community-building activities sponsored by program and agency and encourage participants to support these activities
  • Possess the willingness to work a flexible schedule, including some weekend and/or evening hours as necessary
  • Be aware of programmatic targets and goals and work towards these in all activities provided to participants
  • Engage in professional development and apply learned knowledge towards using evidence-based best practices
  • Perform other tasks as assigned

WORKING CONDITIONS: Typical office setting that will require travel between office and client’s home, and possible multiple sites consistent with a community center environment.


PHYSICAL REQUIREMENTS: Position requires frequent communication with participants, management, and staff. Remain in a stationary position, as well as operate a computer and other office machinery, such as phone and copy machine. Occasionally lift up to 20 lbs.
