I) Job Purpose
Provide access and referral service coordination to families that are a part of Fort Worth Independent School District (FWISD). The FWISD Navigator will work with community partners and FWISD to provide opportunities to children, youth, and parents/caregivers. This includes the assessment of social determinant of health needs, trauma, and developmental screenings. Provide individualized, trauma-informed, culturally, and linguistically competent services based on evidence-based practices for children, youth, and their families.
II) Essential Functions
A) Receive referral from FWISD and works with the MHMR Case Manager/Mental Health Consultant (CM/MHC) to ensure intensive services are available to families.
B) Research and network with referral sources in communities served by MHMR of Tarrant. Accesses ECS, MHMR, community, state and national resource data bases and online live binders for referral sources.
C) Complete assessments with families to ensure referrals are appropriately made both internally and externally.
D) Collaborate with school personnel, including teachers and counselors, to ensure all needs are addressed.
E) Deliver evidenced-based treatment of mental, emotional, behavioral, and substance use disorders and/or conditions. This includes (but not limited to) mental illness, emotional disturbances, substance use, and co-occurring disorders.
F) Direct care services will be provided to youth and families face-to-face.
G) Maintain list of local programs and resources that have capacity to serve families.
H) Assist with updating of databases to include new service programs in the community for families
I) Follow up with families as needed to determine access and service provision.
J) Document contacts and content of contacts as required.
K) Utilize Electronic Health Record (EHR), as appropriate, for admissions and/or documentation
L) Schedule intake appointments for families that are referred for agency services through EHR.
M) Meet as scheduled with Navigation team and supervisor.
N) Attend outside networking and referral source meetings as requested by supervisor.

P) Perform other job duties or responsibilities as requested or assigned.
III) Knowledge of Laws, Policies/Procedures, Skills, and Abilities
• Applicable sections of Texas Administrative Code, Third Party Payer Requirements, MHMR of Tarrant County Policies and Procedures, Mental Health Code, Medicaid Procedures, Compliance Plan, MHMR of Tarrant County Employee Handbook, HIPAA, and applicable Federal standards.
• QMHP demonstrates effective organizational, time management, and communication skills.
• QMHP is self-motivated and self-directed demonstrating the ability to work autonomously.
• QMHP provides excellent customer service both within and outside the agency.
• QMHP demonstrates the ability to engage individuals in services while maintaining appropriate professional boundaries.

IV) Internal & External Customer Service
This position requires extensive internal and external contacts with clients, family members, significant others, inter-agency staff, service providers, and practitioners. The QMHP accomplishes communication with exceptional written and verbal skills, by answering phone calls and emails within one business day, and by conducting themselves professionally in all interactions. Other contacts may include, but are not limited to: John Peter Smith Hospital, Family Services, Social Security Administration, Department of Assistive and Rehabilitative Services (formerly TRC), Department of Regulatory and Protective Services, other area hospitals, local Police Department(s), area United Way Agencies, local housing authority and/or local Transportation Authority.
V) Travel
This position requires driving to and from locations for work and/and may require overnight traveling. Employees are required to have a license in good standing and be able to be covered by agency liability insurance. If personal vehicle is utilized for work travel, then maintaining personal auto insurance coverage is required.

Substitutions
For persons whose bachelor's degree is not in an area designated by Texas requirements as approved, 30 hours of behavioral health coursework must be documented within the transcript to be accepted by the committee as satisfying the education requirement
Job Preferences
Two (2) years' experience in child/adolescent services and training/Knowledge in Trauma Informed Practices
Bilingual-Spanish fluency
License(s)/Certifications
Valid driver's license with an acceptable driving record.
Eligible to obtain Qualified Mental Health Professional (QMHP-CS) certification.
Must provide license (if applicable) and official documentation of all educational attainment i.e. Transcript/Degree/Certificate

All staff are required to participate in agency Emergency Preparedness and Environmental Safety programs and may be assigned by their department as a key/essential staff level function during critical events or for the purpose of sustaining business continuity.
This position may require temporary or permanent re-assignment to any MHMR Tarrant facility as determined by program needs and/or the Division/Director.
Assigned work hours may change as the needs of the agency change.
The Functional Title of this position may change as the needs of the agency change.
All work will be completed within the scheduled work hours. All non-exempt (hourly) employees are expected to clock in and clock out for each work shift, no work should be done off the clock.
Prior approval from supervisor is required for all Paid Time Off (PTO) and Overtime.
MHMR reserves the right to change, add to or eliminate positions as it deems appropriate.
Employment is at will, as well as agency needs may change.
Agency dress code is to be followed at all times.
Regular and predictable work-site attendance, and punctuality is an essential function of this position.
