A DAY IN THE LIFE OF A WRAPS CASE MANAGER
In this position, you will coordinate person-centered services with Minnesotans and caregivers over the phone and in person, working closely with county Adult Protective Services social workers and their APS County Service Plan. You will research and link needed services and resources with at-risk older adults to reduce the opportunity for abuse, neglect, exploitation, and self-neglect.
This is a pilot program expected to run through September 2024.
QUALIFICATIONS
2 years of experience with case management or care coordination in a community setting
Do you have excellent communication and interpersonal skills? Are you highly organized and detail-oriented? Do you have strong problem-solving skills? Can you successfully work independently and alongside a team? Are you great at time management? If yes, you may be perfect for this position!
WORK SCHEDULE
This position is hybrid, working in the community, at home, and in the office Monday - Friday, 8:00 AM - 4:30 PM

QUALIFICATIONS
2 years of experience with case management or care coordination in a community setting
- Bachelor's degree from an accredited college or university in a human services-related field. Social work and nursing degrees are strongly preferred.
- Compliance with all state licensure requirements
- Reliable transportation
- Must reside in MN or WI to be able to travel to meet clients
