Salary: $28.00 - $32.00 DOE

If so, look no further Full Life Care, a Transforming Age Affiliate, is  \looking for Community Transition Specialiststo provide direct service to our community members in need of housing stability, assistance with moving, and other services for our clients currently living in institutionalized settings.  

 

Our Community Transition Specialists 

 

  • Provides assistance and support to ensure referred clients successfully transition from an institutional setting to the community.  
  • Conducts pre-transition consultation and needs assessment with clients 
  • Provides client services necessary to stabilize e  
  • Works with local housing authorities, other housing resources and other area non-profits regarding housing availability, subsidies and voucher capacity. 
  • Coordinates moves using state contracted vendors and getting approvals through DSHS or AAA case managers.   
  • Acts as a liaison between the client, family members and informal supports, state or city case manager, medical providers, housing providers, and relays relevant information to appropriate parties. 
  • Completes reporting requirements such as time sheets, weekly/monthly reports and other documentation required for participants; 
  • Performs other duties as assigned.; 

 

What you need to succeed!:

 

  • A Bachelor’s Degree or higher in Social Work, Psychology, or related field, or the equivalent combination of education and experience.  
  • A minimum of two (2) years’ work experience in social services.  Housing specific experience is ideal. 
  • Current Washington State Driver’s License and proof of safe driving record obtained through appropriate Washington state department; 
  • Must have access to private reliable means of transportation; may require reliable vehicle for client transport; 
  • Ability to effectively use computer and communication technology; Able to use including Microsoft Office Suite: Excel, Word, PowerPoint, OneNote, Teams.  
  • Ability to maintain professional boundaries and practice self-care.
