Coastal Hospice is seeking Full-Time Day Shift REGISTERED NURSE CASE MANAGER to support the Wicomico County Area
Essential Duties and Responsibilities:
- Coordinates with patient/family and Interdisciplinary Team to implement and manage all aspects of patient/family hospice care including physical, spiritual, psychosocial, financial and bereavement needs in the patient’s home or inpatient unit
- Responsible for developing, implementing and evaluating a plan of patient care utilizing ANA (American Nurses Association) Standards and the Nursing Process
- Utilizes equipment and technology skillfully and is technically competent in nursing procedures
- Oversees and/or participates in safe medication handling and practices
- Educates patients and families on significant self-care and end of life issues, all hospice services, patient liability for payment and ensures consent and payer forms are completed
- Acts as a patient advocate. Provides support, information and direct care
- Triages phone calls to identify patient/family needs and notifies Admission Services of referrals
- Oversees and delegates assignments of CMTs, LPNs, Hospice Aides, students and volunteers on shift
- Maintains appropriate licenses and adheres to continuing education obligations
Education and Work Experience Requirements:
- Must be able to provide proof of COVID vaccination
- Licensed and currently registered to practice as a Registered Nurse in the State of Maryland, Delaware or other compact state
- BSN from an accredited college preferred
- CHPN certification expected within two (2) years of employment
