JOB SPECIFICS:
- OSF Medical Group Streator Center for Health has an opportunity for a Family Medicine, Med/Peds, or Internal Medicine physician to join our dedicated primary care team of 3+ physicians and 6 advanced practice providers. Be part of an innovative endeavor to redesign healthcare in a rural setting.
- Office hours are Monday - Friday, 8:00 a.m. - 5:00 p.m. Hospital admissions supported by a hospitalist program.
- Embedded services such as Prompt Care, Anticoagulation Clinic, Complex Care, Diabetic Educator, Nurse Visit Clinic, and Behavioral Health Services to assist in management of patients for best patient care outcomes.
- Light call (phone call only) after hours during the week Monday-Friday with nurse triage assistance. Weekend phone call rotation of 1:18. Newborn nursery and pediatrics call is optional.

The Center for Health provides a full spectrum of medical & social services including primary care, urgent care, cardiovascular services and testing, orthopedics, general surgery, OB, podiatry, rehab services, x-ray, laboratory, respiratory, infusion services, community education, a mobile Care-a-Van bringing healthcare to marginalized areas, and other diagnostic services as well as a freestanding emergency department within the facility.
Qualifications
REQUIRED QUALIFICATIONS:
- Education: Doctorate Licenses/Certifications: Physician Licensed to practice medicine in current state.
- Board certified or board eligible in the appropriate specialty(ies). Obtain and maintain medical staff membership and appropriate privileges.
- Must be eligible as a provider for Medicare, Medicaid, and other federal heath programs.
- Physician shall meet all qualifications to participate in OSF's programs for professional malpractice and other liability coverage. Licensed by the Federal Drug Enforcement Administration and the State of Illinois to prescribe narcotics, if applicable.

- Demonstrates the knowledge and skills necessary to provide patient care aligned to the preferences, goals, and needs of the patients served.
