Field Supervisor // Lead Caregiver

R E S P O N S I B I L I T I E S:
- Supervise a team of field staff and ensure the delivery of high-quality care to patients
- Provide guidance, support, and training to field staff
- Conduct regular performance evaluations and provide feedback to staff members
- Coordinate and schedule patient visits and ensure timely completion of tasks
- Collaborate with other departments to ensure effective communication and coordination of care
- Monitor and maintain compliance with HIPAA regulations and company policies
- Assist with the development and implementation of care plans for patients
- Conduct assessments and reassessments of patient needs
- Provide direct patient care as needed, including assistance with activities of daily living, meal preparation, vital signs monitoring, and dementia care

- Willingness to accept on-call duties for patients/caregivers as well as willingness to provide back-up if caregivers are unavailable
- Ensure a clean and safe environment for patients

E X P E R I E N C E:
- Experience in a supervisory role in the healthcare industry
- Experience in hospice or home care, caregiving, social work, or related fields is preferred
- Knowledge of HIPAA regulations and their application in a healthcare setting
- Familiarity with assisted living facilities and their operations / visiting patient's homes

S K I L LS:
- Strong leadership skills and the ability to effectively manage a team
- Excellent communication and interpersonal skills
- Ability to multitask and prioritize responsibilities in a fast-paced environment
- Attention to detail and strong organizational skills
- Compassionate and empathetic approach to patient care
