The ideal Candidate for the Field Nursing Supervisor will:
- Obtain initial comprehensive assessments.
- Provide post hospitalization and ongoing reassessments.
- Offer patient and family education.
- Develop and update the patient's care plan, LPN/RN, and HHA supervision- annually, 6 months and PRN.
- Troubleshoot with family and clinicians to ensure safe care is delivered to all patients.
- Obtain interim medical orders and must be able to maintain accurate and timely documentation within the electronic system.
- Communicate with the home care team and physicians.
- Assist with orientation and in-service education as needed.
- We provide full PPE for all our caregivers and nurses.
Job Type:

- Mainly field based
Experience & Skills:
- Current and Valid NYS RN License, BSN preferred.
- 3 years of supervisory experience and home care experience.
- Strong Ventilator, Tracheostomy, and G-Tube experience preferred.
- Demonstrated ability to work both independently and along with others.
- Must be able to deliver information in a clear and concise manner.
- Experience with EMR is preferred, but not required.
- Excellent Time Management.
Education:
- Graduate of an accredited School of Registered Nursing with a valid and current New York State License. (Required)
License/Certification:
- RN License (Required)
