Job Description
Fatality Review Specialist
DHHS Office of Service Review
The Utah Department of Health & Human Services, Division of Continuous Quality and Improvement, Office of Service Review, is looking for a dedicated person to work as a Fatality Review Specialist. This position will review fatalities and other critical incidents to understand decisions made and actions taken while keeping the best interest of the clients served in mind. This position will work with other operational units within DHHS as well as our external partners to identify barriers and identify system improvement recommendations by completing interviews and conducting Fatality Review meetings. This position will be responsible for creating reports and presentations to department administration and the legislature.
Preference may be given to:
  • Current Utah Department of Health & Human Service employees.
  • 1-2 year work experience working with vulnerable populations (Health services, nursing home, community center, prison, homeless shelters).

Example of Duties
Principal Duties
  • Conduct Fatality Reviews for people who have died and had services from the department within the past 12 months.
  • Review fatality cases and write case analysis.
  • Conduct debriefing interviews with case managers and providers in a supportive and analytic manner.
  • Synthesize and analyze case reviews and interviews to identify underlying issues for system analysis.
  • Track data and trends to make recommendations and analysis for system improvement.
  • Prepare and conduct safety mapping.
  • Prepare and conduct Fatality Review committee meetings.
  • Help prepare data to complete an annual Executive Summary that is presented to the legislaturePeriodically update the divisions on status of recommendations and report to office and division leadership needed.
  • Other duties as determined by the Fatality Review coordinator in coordination with division leadership.

  • Risks found in the typical office setting, which is adequately lighted, heated and ventilated, e.g., safe use of office equipment, avoiding trips and falls, observing fire regulations, etc.
  • Typically, the employee may sit comfortably to perform the work; however, there may be some walking; standing; bending; carrying light items; driving an automobile, etc. Special physical demands are not required to perform the work.
