Job Title: Family Support Worker (Per Diem, 10-15 hours, Non-Exempt)
Program: Family Support Services
Reports To: Family Support Program Manager
Hiring Range: $19.00 per hour
Job Summary:
Family Support Workers will be responsible for the delivery of services assigned, and the completion of related documentation. This may include a variety of support services such as psychosocial rehabilitation, caregiver and Family Support, community self-advocacy, respite, and prevocational services (depending on level of education and years of experience) In collaboration with the Care Manager and family they design appropriate goals for the child to live successfully in their homes and communities. Service Counties include Saratoga, Warren, Washington.
Duties and Responsibilities:
Provide services based on the child’s needs as identified in the Plan of Care and Treatment Plan
Coordinate with the child and caregiver on the scheduling the provision of services
Work with family members to identify goals and objectives of treatment
Facilitate service provision and attend relevant meetings as required
Implement strategies that include planned activities, structured environments, and organized recreational and social activities
Assist individuals and families in identifying personal strengths and resources for positive change
Act as a resource for children and their families
Complete written documentation
Complete all Mandated trainings
Maintain on going regular communication with the Program Manager and families.
Qualifications:
- Must be at least 18 years of age and have experience working with children and families (preference given to those with experience working with children with special needs);
- Must have a minimum of a high school education (a high school diploma or G.E.D.),
- Must have a Bachelor's Degree in a related field for specific services
- Must be cleared by the State Child Abuse Registry; and complete fingerprinting for a criminal history background clearance
- Must possess a valid and clean NYS Driver’s License and provide evidence of appropriate insurance coverage.

Excellent organizational and interpersonal skills
Good communication skills and proven ability to work with special needs children and their parents.
Ability to work collaboratively with a multi-disciplinary team
Ability to work with all ages.
Able to work independently and meet state and federal mandated deadlines
Knowledge of community resources.
Positive Image for CAPTAIN and a dedicated client skill builder.
Some Computer experience required
Telecommuting and Remote Work:
Generally, work from home is not a viable option as this position requires significant in-person presence. Working from home will be considered in unusual circumstances and only when the Executive Director approves such a request.
