Job Duties

The Infant & Toddler Connection of Virginia is Virginia's early intervention system for infants and toddlers (age 0-36 months) with disabilities and their families. Any infant or toddler in Virginia that is not developing as expected or has a medical condition that can delay typical development is eligible to receive early intervention supports and services under Part C of the Individuals with Disabilities Education Act (IDEA). Early intervention supports and services focus on increasing the child's participation in family and community activities that are important to the family. In addition, supports and services focus on helping parents and other caregivers know how to find ways to help their children learn during everyday activities.

This position is a part of the Infant Toddler Connection (ITC) of Central, Virginia, serving Amherst, Appomattox, Bedford, Campbell, and Lynchburg. This position will provide Service Coordination (Case Management) in the natural setting working with children (ages 0-3) and their families. The ITC program implements the early childhood coaching approach, which focuses on building the caregiver's capacity to enhance the child's development using everyday interactions and activities. Service Coordinators are responsible for documentation using an electronic health record system to include, but not limited to Quarterly Reviews, IFSPs, Annual Reviews, and progress notes. Must completed all Infant Toddler Connection of Virginia Early Intervention modules and pass the competency tests for each with 80% accuracy. Must be willing to become Endorsement in Infant Mental Health

 

Provides guidance to clients with a knowledge of social and vocational case
management and counseling methods to develop individualized service plans and
perform psycho social assessments to determine personal, social, health and
economic needs. Assists clients by independently identifying and referring them to
available federal, state, local, and private resources to assist with their needs.
Educates clients on clinical and other relevant procedures and coordinates client care
and community resources.

 

Minimum Qualifications

Experience and knowledge of: family services principles, practices and ethics;
development of treatment plans to deliver services and locating and securing
resources; knowledge of infant and child development; parent-child
dynamics; child abuse, neglect, and trauma impact
Experience and knowledge of: individual counseling and relationship building;
effective communication; group leadership; documentation, data entry, and
record keeping
Ability to promote appropriate programs; be a positive role model and
mentor for clients, providing emotional support, health and safety
information, and community referrals
Strong verbal and communication skills; skill in interviewing to gather,
interpret, and use data appropriately; case management and excellent
organization skills; ability to handle multiple tasks and work effectively as a
team member
Ability to adhere to confidentiality and handle sensitive information
Computer skills to include web-based case management documentation
