Position Overview - Family Support Partner - Care Coordinator
Provides care coordination services to clients, including but not limited to transportation, referral, vocational and psychosocial education, and linkage with community resources and supports. The Family Support Partner provides a high level of customer services to referring parties by:

  • Promoting a timely access to appropriate care
  • Maintaining ongoing tracking and appropriate documentation using the required technology and reports as may be required by SLVBHG administration
  • Ensuring patient/clients barriers to receiving services are addressed
  • Communicating status of referrals back to referring party in a timely manner

Essential Job Functions - Family Support Partner - Care Coordinator
  • Assist patients/clients in problem solving potential issues related to the behavioral health system, medical health care system, financial or social barriers;
  • Complete necessary health assessments to determine appropriate needs and linkage.
  • Assemble information concerning patient/client's whole health needs and referral needs along with identify community resources for patient/client, family/caregiver(s) and community agencies.
  • Interface and collaborate with other community agencies and resources to optimize service.
  • Provide coordination and linkage to services appropriate to the age-specific needs of all patients/clients in accordance with the principles of growth and development for infant through geriatric life cycles.
  • Build professional relationships and coordinate care with internal and external medical providers to ensure quality care and follow through
  • Consults and advises with other health care team members on methods of assisting patient/client and their families in overcoming social and emotional difficulties which may prevent effective health care, this may include psychoeducation, brief interventions or referral for further treatment
  • Provide behavioral health outreach to specific populations
  • Be the point of contact for partnering agencies in the community to collaborate within specific needs of entire population
  • Collect and interpret data on a regular basis to determine focus and direction of individual patient/client needs
  • Participate in multidisciplinary staffing
  • Conduct all professional activity in an ethical manner, maintain records accurately, adhere to timeliness in accordance with Center's standards, follow applicable policies and procedure, consult internally and externally with regard to patient care
  • Adapt to change in the workplace and use change as an opportunity for innovation and creativity

Requirements
Education/Skill Requirements - Family Support Partner - Care Coordinator
  • BA in human service or related field
  • 3-5 years' experience in clinical or community resource settings; care coordination and/or case management experience is desirable
  • Proficiency in communication technologies (email, cell phone, etc.)
  • Highly organized with ability to keep accurate notes and records
  • Local knowledge about and connections to community health care and social welfare resources is desirable.

  • Must possess the ability to communicate with other in a clear, understandable and professional manner on the phone and in person; and demonstrated use of good written and verbal and nonverbal communication skills

  • Must Acknowledge client's rights on confidentiality issues, maintains client confidentiality at all times, and follows HIPAA guidelines and regulations.

Work Environment: General office environment; light physical work as needed: travel or errands outside the office.
