Job Description:
Facilitates patient and family access to appropriate hospital and community resources.Pay Rate : $21.65 + Depending on Experience
Shift : 9:00am - 5:30pm (Monday through Friday)
Department: Primary Children's Hospital - Salt Lake City Campus
Additional Details: 3 years Healthcare Experience. Spanish speaking preferred.
Job Essentials
- Assists families to identify options to meet their various resource and support needs in collaboration with social workers and other hospital departments.
- Partners with staff members to ensure that appropriate resources are made available to patients and families (i.e.: meal vouchers, hygiene products, clothing, etc).
- Networks with community agencies and external services to maintain current information regarding resources and support.
- As a member of the Destination Services team, acts as a liaison between Primary Children’s Hospital, Ronald McDonald House, and other external housing support.
- Utilizes appropriate hospital staff to help meet patient and family needs and remains familiar and up to date with the services that Primary Children’s Hospital offers.
- Connects families with staff, departments, or services in the hospital, as needed.
- Participates in team coverage as demand arises, requiring flexibility to adapt to the program's needs.
- Partners with Volunteer Services and the Intermountain Health Foundation to maintain and distribute Family Resources, and community donations (i.e., Tucker Totes).
- Coordinates the Family 2 Family Mentorship Program, which includes leading a steering committee of volunteers and staff. Additional duties include recruitment, screening, selection, training, and oversight of 10-20 volunteers. Leads and coordinates outreach efforts and presents curriculum and training for Family 2 Family mentors. Provides curriculum for providers and staff specific to utilization of the Family 2 Family Mentorship Program. Coordinates with outpatient clinics and inpatient services as part of the Destination Services Team to offer families traveling from out of the area assistance with lodging, transportation, and other hospital services, as needed.
Minimum Qualifications
- Presentation, public speaking and/or education experience.
- Computer Skills, i.e. word processing, spreadsheets, databases and internet.
- Customer Service Skills.
- Facilitation Skills. Problem Solving Skills.
Preferred Qualifications
- Bachelor's Degree from an accredited institution (degree will be verified).
- Healthcare Experience
- Assisted Social Work Experience.
- Experience working with parents or families of hospitalized children.
- Experience coordinating volunteers.
