Role Description:
We are seeking a Nurse Practitioner with excellent communication, clinical leadership, and interpersonal skills to provide exceptional services to a diverse adult patient population in underserved communities throughout Chicago. Vivia Health is centered on the collaborative care model, with a focus on preventative care, disease management and mental health. The care team is composed of the Nurse Practitioner, psychiatric Nurse Practitioner, therapist, case manager and nutritionist. In this role, you will be responsible for providing direct patient care and working collaboratively with our interdisciplinary team of healthcare professionals to build meaningful patient relationships through care planning.
Core Responsibilities:
- Provide exceptional primary and preventative care to all patients.
- Conduct initial assessments for newly discharged patients with mental health and substance use diagnoses.
- Perform medication reconciliation to refill or prescribe appropriate medication regimen based on assessment and diagnosis.
- Refer patients to appropriate care team or specialist for follow up care.
- Administer annual wellness visits and health risk assessments.
- Educate patients on their physical and mental health conditions, treatments, and care plans.
- Chronic disease health care management and education.
- Order, perform, or interpret the results of diagnostic tests and lab work.
- Proficient in phlebotomy for basic lab work.
- Recommend interventions to modify behavior associated with health risks.
- Counsel patients about drug regimens and side effects or interactions with other substances, such as food supplements, over-the-counter medications, or herbal remedies.
- Provide patients with information needed to promote health, reduce risk factors, or prevent disease or disability.
- Participate in Vivia Health promotional activities.
- Detailed documentation.
- Transparent communication with staff and management.
- Other duties as assigned.
Required Qualifications:
- Graduate of an accredited school of nursing.
- Master of Science in Nursing, Doctor of Nursing Practice
- National certification as a Nurse Practitioner through AANP or ANCC
- Licensed to practice in Illinois with current Illinois registration and in good standing.
- Active control substance and DEA license preferred
- Record of prior stable employment and/or favorable references.
Preferred Qualifications:
- Bilingual Spanish or Arabic speaking
- 1 to 2 years' experience as Nurse Practitioner in primary care
- Experience or exposure with mental health and substance use disorders
- Desire to be a part of an innovative collaborative care team model

- Passion for educating patients and families
- Knowledge of Athena and Microsoft Word Suite
Desktop Computer Skills
Using a computer application to manage large amounts of information, including creating and editing simple databases, inputting data, retrieving specific records, and creating reports to communicate the information. Using scroll bars, a mouse, and dialog boxes to work within the computer's operating system. Being able to access and switch between applications and files of interest.
