We are a Mobile Clinic that visits patient's home, board and care, assisted living facilities and skilled nursing.
Candidate will do the following :
- You will be assigned to a City of your choice or near your Home/ Work covering multiple locations and working with different teams to manage patients at their homes
- You will deliver proactive preventive care at patients' homes to reduce unnecessary hospitalizations visits.
- Partner with the patient's primary care provider and specialists to deliver integrated care
- Diagnose and treat patients, consistent with standard of care
- You will be conducting comprehensive exams and creating care plans for the most vulnerable, senior elderly Medicare patients
- Providing appropriate education and counseling to patients and their families/caregivers
- Documenting appropriately in EHR and other systems
- Collaborating with other members of the care team to manage the needs of the patient
This is a fun and rewarding type of Job. If you don't have time to do your online notes we can help you do it. We have a medical transcriber standing by to help you so you can stay focused on your patient, family and friends at the end of the day.
Job Types: Full-time, Part-time, Contract
Pay: $140.00 - $160.00 per hour
Expected hours: 3 – 35 per week
