Job Description
We are seeking a responsible Nurse Practitioner to become a part of our team and can start ASAP! You will focus on providing high-quality patient care as part of a Medical Group Team. Flexible scheduling is offered where providers can flex their time over the 7-day work week.
Work Locations available in Tulare County Area CA
Responsibilities:
- Conduct in-home assessments including evaluation of past medical history, review of systems, medical reconciliation, vital signs, and comprehensive physical exam
- Identify diagnoses to be used in care management and active medical management in the furtherance of treatment
- Perform evidence-based practice screenings including point-of-care testing (as appropriate)
- Formulate a list of current and past medications using clinical knowledge and judgment and the findings of your assessment
- Facilitate referrals to other healthcare professionals and medical facilities
- Educate patients on topics such as disease process, medication, and compliance
Qualifications:
- Active NP license or ability to obtain by start date. Licensure must be unencumbered, free of any open/unresolved disciplinary actions including probation or restrictions against privilege to practice.
- Ability to perform physical requirements such as sitting, standing and kneeling to perform physical assessment to patients.
- Reliable transportation to complete home visit assessments
- Full COVID-19 vaccination. Candidates should adhere to all federal, state and local regulations as well as all client requirements and will obtain necessary proof of vaccination, and boosters when applicable before employment to ensure compliance
- Excellent written and verbal communication skills
- Strong leadership qualities
