Description
The Hennepin County Public Health Clinic (PHC) is seeking a Family Nurse Practitioner to provide medical screening and services to newly arrived refugees as well as provide assessment and treatment with active tuberculosis (TB) and latent TB infection (LTBI). Care is provided across the lifespan, infancy to adulthood, within a multidisciplinary team of medical providers including physicians, Nurse Practitioners, registered nurses, medical assistants, and community health workers. The Family Nurse Practitioner may also provide sexual health care services including HIV screening and sexually transmitted infection (STI) testing and treatment, as warranted by clinic volume and utilization.

Location and hours:
This position is located at the Public Health Clinic at 525 Portland Avenue, Minneapolis, MN 55415. Work hours will typically be Monday through Friday, 8 a.m. to 4:30 p.m., and will require working one evening shift per week once the clinic resumes evening hours. No weekend, overnight, or holiday shifts are required.

In this position you will:
  • Conduct refugee health screening exams, including health history, physical examination, and preventive screening of newly arrived refugees to identify acute and chronic health problems as well as identify public health risks.
  • Develop and maintain positive relationships with community service providers familiar with the needs of refugees.
  • Evaluate and treat patients with active and latent tuberculosis, including medication management.
  • Provide health education, counseling, and consultation and make appropriate referrals.
  • Collaborate with the Minnesota Department of Health (MDH) refugee health program to ensure compliance with screening requirements and provide clinical expertise.
  • Provide sexual health care services including comprehensive HIV testing, STI testing and treatment, family planning, and HIV prevention, dependent on clinic staffing needs.
  • Work with a diverse, interdisciplinary team to deliver health services, develop team goals, performance outcomes, and effective referral coordination.
  • Complete accurate and timely documentation in Epic, the Electronic Health Record (EHR).

Need to have:
  • One of the following:
    • Possession of a valid license as a Registered Nurse issued by the State of Minnesota and National Certification as a Nurse Practitioner (CNP).
    • Possession of a valid license as an Advanced Practice Registered Nurse issued by the State of Minnesota.
  • National Certification as a Family Nurse Practitioner (FNP).
