The Family Nurse Practitioner (FNP) will be responsible for:
- Performing interviews, health histories, physical examinations, mental status assessments, and diagnostic tests to determine diagnosis and current health status of clients.
- Providing medical crisis intervention and counseling as required by direct visitation or telephone contact.
- Completing evaluations, medication reviews, care plans, and progress reports.
Qualifications:
- Certificate and current registration to practice as a Nurse Practitioner in New York State.
- At least one year of experience providing direct services in a family medicine setting.
- Ability to effectively use an EMR and complete all required documentation in a timely, professional manner.
- Salary: $120-140K
