THIS IS A GRANT-FUNDED POSITION
JOB SUMMARY/ESSENTIAL JOB FUNCTIONS: Under the direct supervision of the Program Manager, the Family Advocate Counselor exists to support and assist caregivers in determining specific needs and obtaining needed support with projects resources through the network and /or other providers. This position will allow hands on contact with individuals and families through home and office visits.
DUTIES AND RESPONSIBILITIES:
- Identifies participants eligibility through intakes daily via phone, walk-ins, and referrals from agencies.
- Conducts home visits to assess family eligibility and environment-include monthly and /or quarterly contacts.
- Develops an individualized Action plan- an Action plan is developed in collaboration with the caregiver for the family to receive appropriate services.
- Determines specific needs with families.
- Completes family re-determination, case graduation and case closures.
- Provides monthly or quarterly face-to-face meetings or phone contact with program families.
- Submits accurate monthly reports denoting the number of client contacts and activities, successes, and challenges.
- Drives and shops for food, clothing, and other essential items for families in the program. Delivers these items when necessary.
- Maintains monthly state requirements in TFACTS Database.
- Collaborates with outreach agencies to secure additional resources to gain greater family stability.
- Performs other related duties as assigned.
MINIMUM REQUIREMENTS:
EDUCATION: Bachelor's Degree in Social Work or related field. (TRANSCRIPT REQUIRED)
EXPERIENCE: Two (2) years of experience working with families and children in crisis and diverse families. (Experience with facilitating support groups is preferred).
KNOWLEDGE, SKILLS, AND ABILITIES:
- Ability to facilitate and organize groups and program activities.
- Knowledge of community and social services resources.
- Ability to perform home visits. Must be able to work closely with families and program team members in a positive manner.
- Knowledge of basic computer skills (Microsoft Word, Power Point, Access, Excel, etc.).
- Knowledge of local and state community resources and services.
- Ability to work with families and children in crisis.
- Ability to be empathic and a good listener.
- Ability to be able to identify unsafe environments that might put families and children at risk.
- Ability to facilitate and manage group sessions.
- Ability to work independently, efficiently and to accomplish both routine and detailed tasks.
- Ability to be prompt, detail-oriented, courteous, efficient, and professional.
- Ability to adapt to the changing policy and environment that is indicative of the University and state mandates.
- Ability to work with others in conjunction with the University, Department of Children's Services, program vendors and families.
- Ability to meet the monthly, quarterly, and yearly goals as set forth by the Program Administrator, University and DCS.
- Ability to maintain confidentially and professionalism as it concerns information about staff, families, and program matters.
LICENSE/CERTIFICATION: Valid Driver's License required. (COPY OF LICENSE/CERTIFICATION REQUIRED)
WORK SCHEDULE: This position frequently works weekends and evenings. This position requires travel within Shelby County, with occasional overnight travel.
