Summary
The Wraparound Facilitator position is full-time and will provide services in the Northstate (Butte, Sutter, Yuba and Shasta counties). This position facilitates a family-centered, strength-based, culturally sensitive, needs-driven planning process for creating individualized services and supports for children, youth, and their families. Ensure the provision of Intensive Care Coordination (ICC) that is responsible for facilitating assessment, care planning, and coordination of service, including urgent services, for children/youth who meet the Katie A. subclass criteria. The Wraparound Facilitator assures access to natural, professional, and community-based options, activities, and opportunities. This position includes coordinating services, facilitation, and documentation along with all other functions and duties that are assigned.
Education and/or Experience

Education and/or Experience
- A Master’s Degree in Behavioral Sciences or a BA degree in health or human services-related field from accredited college or university and at least two (2) years of experience with ED children, adolescents, and families
- Possess a valid State of California Driver License in good standing
The estimated base pay for the Facilitator is $19.00 to $26.00; additional individual compensation may be available. Base pay is only a portion of the total rewards package, and a comprehensive benefit program is available for qualifying position.
