Hours: 10-15hrs/week

Compensation: $30/hr

The Engagement Coordinator works in a multidisciplinary team setting with a Program Manager, Care Navigators, and direct service providers. Their core function is to establish and maintain opportunities for enrichment and engagement among a specific population of older adults.

Duties are largely performed on-site within a large apartment building working with a group of 12-15 professional women who are part of a religious community. Performs all duties in a manner that fosters the achievement of the organization’s mission to identify choices and provide services to help people of all ages, abilities, and incomes to live at home.

Most residents are retired with varying levels of care needs. They live in individual units within a large apartment building and gather as desired in a common space for meals, socializing, and scheduled meetings and activities.

Requirements:
  • Co-creates and facilitates the implementation of individual and group social and educational events, reflective of the needs and preferences of residents on a predictable schedule.
  • Meets with residents one-to-on to determine individualized needs and preferences.
  • Identifies and arranges for enrichment opportunities onsite and in the community in groups and 1:1.
  • Coordinates with area universities to build internship/engagement opportunities that support enrichment and wellness goals.
  • Serve as a supportive communication hub to ensure events, meetings and announcements are conveyed and barriers to participation are addressed.
  • Escalates health and wellbeing concerns and/or direct service provider quality and scheduling concerns to navigator and, if appropriate, Program Manager, according to Sage Solutions’ escalation protocol.
  • Updates Sage Solutions Point Person prior to weekly meetings with Account Leadership and participates as requested.
  • Participate in on-call rotation for after-hours emergency or hospitalization care and support needs.
  • Meet with Program Manager and Care Navigators on a regular basis.
  • Assists in fulfilling responsibilities of Program Manager during any absence.
