Qualifications:
- Must be 21 years of age.
- Possess a valid Nebraska Driver’s License, be insurable, and have a good driving record.
- Minimum of an associate's degree in psychology, sociology, or related human service field, with a preference for a bachelor's degree.
- Strong interpersonal skills, including verbal fluency, empathy, and focus on others.
- Ability to form a working alliance with a broad range of clients.
- Knowledge of supportive services and referral agencies in the community.
- Excellent written and oral communication skills.
- Ability to maintain strict confidentiality following 42-CFR and HIPAA regulations.
- Resourceful, confident, assertive, with strong boundaries and problem-solving skills.
- Effective team player with exceptional time management and stress management skills.
- Flexible schedule, available on evenings, weekends, and holidays as needed.
- Non-judgmental, caring attitude toward clients.
Duties and Responsibilities:
- Complete required trainings and certifications within established timeframes.
- Maintain a caseload of up to 15 Emergency Community Support clients.
- Conduct 72-hour follow-up contacts with residential clients.
- Complete orientation and intake documentation with all clients.
- Complete authorizations for services through the Centralized Data System (CDS).
- Provide direct rehabilitation and support services to each client in the community.
- Assist clients in accessing available resources, including housing, vocational, and educational assistance.
- Attend court appointments and psychiatry/medical appointments with clients.
- Teach skills including communication, budgeting, coping, relapse prevention, and healthy meal planning.
- Coordinate services and provide support for clients in crisis.
- Track Community Support department contacts and ensure billing requirements are met.
- Complete audits of Outpatient billing each month for accuracy.
- Ensure Community Support providers obtain and maintain medication aide certification.
- Monitor CDS system for accuracy and completeness of outpatient services.
- Answer business calls and conduct general business correspondence.
- Assist with outpatient flex fund and housing fund requests and related activities.
- Conduct regular chart reviews and submit to the Outpatient Director.
- Complete TCU surveys with clients regularly as assigned by the Operations Officer.
- Track and monitor all Community Support and Intensive Outpatient authorizations.
- Monitor Community Support productivity and report to the Outpatient Director weekly.
- Ensure coverage is in place for all duties when unable to perform.
- Conduct contingency management drug screens when available.
- Complete monthly inventory and submit supply order requests.
- Conduct all Health and Safety drills as assigned.
- Ensure client demographic information is updated in inSync.
- Oversee office maintenance and cleanliness.
Probationary Period: 6 months
