Job Title: DYCD- Youth Health Worker

Department: Prevention

Reports To: Health Navigation Program Coordinator

Location: Wellness Center: 3144 Third Ave

Job Summary:

The Youth Health Worker will be part of the 3rd Ave Drop-in program, a space created for LGBTQIA+ youth and young adults ages 16-24, which aims to raise awareness and education to help fight stigma around HIV and is a key position within our Health & Navigation program. Under the direct supervision of the Health Navigation Program Coordinator, the Youth Health Worker will utilize a status-neutral approach, promote and support integrated and person-centered models of medical and behavioral health care and increase the availability and improve access to culturally and linguistically affirming Prevention Services.

Primary Duties & Responsibilities include, but are not limited to:

· Provide affirming and up-to-date health education on topics that include, but not limited to condoms and safer sex products, PreP, Emergency PEP, iART, HIV care and treatment, U=U, HIV/STI screening, reproductive health, mental health, substance use and supportive services

· Utilize a client-centered, harm-reduction framework service model to determine needs and develop a client-centered service delivery plan that support the client in making their own autonomous choices about their health care, well-being, and sexual health needs

· Maintain the 3rd Avenue Facebook fan page and other social media outlets for program promotion and HIV prevention education

· Implement, facilitate and report on monthly 1 day awareness events that support ETE and NHAS goals

· Lead daily structured health & wellness HIV prevention based discussions and socialization activities as scheduled from the drop in calendar

· Provide targeted HIV/HCV/STI point-of-care testing, counseling, referral and confirmatory HIV testing to clients testing preliminary HIV-positive

· Complete partner services and NYS provider report form on all new HIV diagnoses as well as previous HIV diagnosis new to the program

· Ensure that all HIV and HCV positive clients are offered and connected to Linkage Navigation Services to provide iART and initial HIV primary care support services

· Assist with wellness checks of drop-in participants utilizing a client-centered service delivery plan that support the client in making their own autonomous choices about their health care, well-being, and sexual health needs

· Assist in the creation of the monthly activities calendar and help maintain space for program delivery

· Greet and welcome new members, provide orientation of services, introduce them to existing members and ensure they are integrated within program

· Complete intakes on all new drop in members, including safety, code of conduct and related program forms

· Monitor daily drop in space activity and drop-in client flow, ensure clients adhere to community safety guidelines and program protocols

· Complete all chart documentation, recording of statistics, returning telephone calls to client and other agencies providing services to clients

· Refer 100% of all clients to other high impact prevention programming and essential supportive services, such as PrEP/nPEP, Linkage & Navigation, Medication Adherence, Mental Health Services, STI testing etc…

· Attend training seminars, case conferences, staff meetings and data driven supervision

· Meet at least 90% of monthly and annual targets as stipulated in the contract work-plan

· Provide excellent customer service at all times, to internal and external customers.

· Clearly and accurately document client charts

· Attend and participate in local community events as required.

· Other duties as assigned by the Prevention Department leadership

Qualifications/Criteria:

· Knowledge of HIV/HCV/STI infection, transmission, prevention

· Experience working with LGBTQ youth and young adults

· Excellent verbal and written communication skills, including strong interpersonal and organizational skills

· Strong computer skills and knowledge including MS Word, Outlook, Excel, and other software.

· Bilingual in Spanish a plus

· Understanding of culturally diverse needs of various ethnic and socioeconomic groups

· Excellent organizational, interpersonal, written and communication skills required

· Willing to work evenings and some weekends

· Knowledge of community resources.

· We follow the NYC Workplace Vaccination requirements.

Education:

· Associates degree with at least one year of relevant experience preferred; HS Diploma or equivalency required with at least 2+ years of relevant experience.
