Discharge Planner/Social Worker

PURPOSE STATEMENT: 

Maple Heights is a 120 bed behavioral health facility, that opened in January 2023. We are seeking a social worker or human services professional to provide support to our clinical team by developing a documented continuing care plan for patients that will ensure ongoing support in recovery and/or behavioral health follow up after discharge from the facility. 

ESSENTIAL FUNCTIONS: 

• Develop continuing after care plan, based on recommendations from the treatment team to promote long term health and relapse prevention.   
• Responsible for setting discharge appointments for patients, identifying community resources, sending referrals, and seeking placement for patients as needed.  
• Provide education to patients on community resources to support successful discharge. Provide all needed documentation to external providers to facilitate the continuum of care.
• Develop relationships with community partners and act as a liaison between referral sources and agency.
• Ensure all patient discharges planning documentation is clear, concise and timely.
• Assist with filing involuntary court paperwork and hearings as needed. 

OTHER FUNCTIONS:  

• Perform other functions and tasks as assigned. 

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS: 

• Associates degree in nursing or human services field required.  Bachelor's degree in social work or human services field preferred. 
• One or more years’ experience in a clinical setting preferred. 

• One or more years’ experience in discharge planning preferred. 

LICENSES/DESIGNATIONS/CERTIFICATIONS:  

• If applicable, current licensure as a LPN, RN within the state where the facility provides services. 
• CPR and de-escalation and restraint certification required (training available upon hire and offered by facility.   
• First aid may be required based on state or facility requirements.
