Discharge Coordinator

```Overview:

Day shift position, Monday-Friday 8-4
We are seeking a highly organized and detail-oriented Coordinator to join our team. As a Coordinator, you will play a crucial role in supporting the efficient operation of our organization. This position requires strong communication skills, the ability to multitask, and a passion for providing excellent patient service.

Responsibilities:
- Coordinate and manage patient cases, ensuring all necessary documentation is complete and accurate
- Utilize knowledge of HIPAA regulations to maintain patient confidentiality and privacy
- Collaborate with healthcare providers to develop and implement effective care plans for pediatric patients
- Use ICD-9 and ICD-10 coding systems to accurately document medical procedures and diagnoses
- Assist with discharge planning, ensuring patients receive appropriate follow-up care
- Maintain medical records in accordance with organizational policies and procedures
- Provide administrative support, such as scheduling appointments and managing phone calls
- Collaborate with behavioral health professionals to support patients' mental health needs

Experience:
- Previous experience in case management or a related field is preferred
- Familiarity with HIPAA regulations and their application in a healthcare setting
- Knowledge of pediatrics and the unique care needs of pediatric patients
- Proficiency in ICD-9 and ICD-10 coding systems
- Experience with inhalation therapy is a plus
- Strong understanding of behavioral health principles and practices
- Excellent customer service skills, with the ability to effectively communicate with patients and their families
- Strong organizational skills, with the ability to prioritize tasks and manage multiple responsibilities effectively

Job Type: Full-time

Pay: $20.00 - $25.00 per hour

Expected hours: No less than 40 per week
