Director Of Social Services

Salary: $80K (BASED ON EXPERIENCE)

Job Description:

· The right candidate will be an out of the box thinker with strong patient advocacy skills and a desire to make a difference in the lives of our residents.

· Plans, controls and directs the services of the Social Service Department.

· Monitors resident progress and collaborates with the Interdisciplinary Team to ensure appropriate and high-quality services to residents.

· Coordinates discharges and ensures residents are appropriately discharged with ancillary services provided and planned.

· Participates in the development and maintenance of resident care plans.

· Is responsible for timely documentation if initial assessments, interim notes, progress notes and completion of the MDS.

· Serves as a resident advocate

Qualifications:

· Must have Bachelors degree, Masters degree preferred

• Minimum two (2) years' experience as a social worker in a healthcare-related setting
• Experience within a nursing home or rehabilitation center is strongly preferred
• Knowledgeable of the rules, regulations, and guidelines that govern nursing care facilities
• Must have knowledge of care plans and MDS requirements

· Communication skills with the ability to deal tactfully with personnel, residents and family members
