SUMMARY OF POSITION: This open position is for Monday - Friday, 1st, 2nd, and 3rd shifts working with all Male Youth population.
Saturday and Sunday 12 hour shifts available.
Staff must be able to work 40 hours each week Monday thru Friday. Staff must have reliable transportation. The position is located in the a MH Youth Group facility. Direct Support Professional provides support to persons in their daily home routine. The DSP provides support for habilitative and rehabilitative activities, personal care, health care, and other activities as outlined in the Plan of care/Person Centered Plan/Individualized Support Plan, with all services provided within the framework of person-centered an informed choice philosophies. must possess the following:
The Direct Service Professional is responsible for assisting the youth mental health population to prepare, to the greatest extent possible, for day to day living. This is to be accomplished by the provision of an environment consistent with quality care of residents.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Adhere to Individual Support Plans/Person Centered Plan/Plan of Care as required to support individual in completing daily living activities including, personal care, home care, and maintenance, and transportation.
- Participate in implementation of person-centered plan under guidance from a Qualified Professional as well as serve as an active participant in the client's treatment team.
- Oversees day-to-day operations of the home including supervision of clients, provides direct interventions to build socialization, behavior management skills, and daily community living skills.
- In accordance with the person-centered plan, the DSP provides direct interventions to promote adaptive skill learning, community inclusion, and development of leisure time interests/activities.
- Maintains documentation on a daily basis, completes daily progress notes in a timely manner, and other data as needed, including incident and accident reports.
- Communicates daily and as needed with other staff and supervisor on progress, needs, concerns and changes related to residents being supported.
- Responds to individual safety needs, including reporting significant changes in behavior and health.
- Provides high quality customer service, serving as a positive representative for the agency with other professionals, families, and the community at large.
- Performs housekeeping duties as assigned including meal preparation in accordance with individual dietary needs and preferences, ensuring residents are included in meal preparation per the care plan.
- Completes medication management activities including assisting residents to take medication, educating residents about medication, correct and timely documentation of medication administration on the appropriate Medication Administration Record and documenting any incidents related to medication.
- Work collaboratively with internal and external team members to ensure residents supported receive most appropriate level of care and the health and safety and client rights are maintained to the highest degree possible.
- Attends regular group or individual supervision as required, including staff meetings, trainings, etc.
- Participates in ongoing Quality Improvement activities.
- Maintains regular attendance and punctuality, manages use of time, and follows all policies and procedures regarding time off.
- Adhere, at all times, to the policies and procedures of Quality Care Solutions, Inc.
- Performs related duties as requested
- To take responsibility for a caseload of clients; contributing to the development of individual care plans; making appropriate risk assessments; working to complete them and helping people with dementia to achieve their potential and maximize their skills.
- To review and monitor the progress of youth clients in line with their care plans and report and recommend changes to activities as appropriate.
- To contribute to the high quality and appropriate care, support and activities are delivered as planned, to the standards required by the QCS and NC DHHS.
- To advise and support careers as appropriate, working in partnership with them to assist in good decision making e.g. which kind of activities would be beneficial for the client.
- To promote and maintain a good standard of care practices within the group home.
- To assist in the daily preparation of the group home, ensuring the premises are safe and in comfortable.
- To ensure clients are ready to safely and comfortably use transport provided and act as an escort where necessary. To ensure that assistance with client personal care is provided with dignity and respect.
- In line with internal and external standards, ensure dietary requirements are understood and any food or beverages are provided appropriately and carefully.
Qualifications: High School Diploma or equivalent form an accredited institution by the state Department of Education.
- 5 years of experience in a MH group home setting working with the youth population.
- Must have a valid NC Drivers License, Valid and current Auto Insurance, and a good driving record.
- Must not have any record of convictions or felonies or substantiations of abuse, neglect, or exploitation.
- Adhere to the QCS, Inc. Drug/Alcohol Policy.
- Must be able to work a flexible schedule, including extended periods of time.
- Must successfully complete all phases of pre-service and on-going training, including, but not limited to, physical restraint training, First Aid, CPR, Universal Precautions, and medication administration.
- Must be able to work in high pressure, high stress environment.
- Position may experience verbal and/or physical aggression from client population.

Salary: $12.00 - $14.00 per hour

- High school or equivalent (Required)
Experience:
- Adult DD/MH Group Home: 5 years (Preferred)
License/Certification:
- NCDL (Required)
