Job Description Key Responsibilities Responsible for the provision of nutritional care to patients/residents from different populations and/or disease states; including the provision of nutrition education and implementation of discharge planning for continuity of care Timely initial and periodic assessments of nutritional needs of patients/residents Participation in performance improvement activities Acts as a member of the interdisciplinary disciplinary team Responsible for supervising and coordinating Clinical Activities and for ensuring continuous provision of nutritional care to patients/residents in the Hospital & Nursing Facility Responsible for performance improvement activities and for insuring continuous provision of nutritional care to patients/residents as well as continuity of P.I. activities of the Hospital and Nursing Facility Assists in the planning and directing of nutrition services activities Performs on-going monitoring of Level III Dietitians and coordinates with Clinical Nutrition Manager on monitoring clinical staff competency Maintains continuous daily communication with the clinical staff on status of unit activities.
Initiates appropriate actions, if necessary Onboarding and Preceptor of dietetic interns and coordination of schedule Attends departmental meetings and with other departments, as needed Completion of meal rounds to ensure conformance to prescribed diets, adequate food intake and overall meal tolerance Performs other related duties as assigned Minimum Qualifications For appointment to Assignment Levels III and IV 1. Registration by the CDR as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist. Special Note for Dietitians assigned to Post-Acute Care (Long-Term Care) facility: Under the Medicare and Medicaid Programs, Reform of Requirements for Long-Term Care Facilities, as outlined in Section 483.60 of Title 42 of the Code of Federal Regulations all newly-hired Dietitians, Levels I-IV assigned to a Post-Acute Care (Long-Term Care) facility, are required to have New York State Certification, issued by the New York State Education Department (NYSED), as a Dietitian-Nutritionist at the time of hire.
Dietitians, Levels I-IV hired prior to November 28, 2016, assigned to a Post-Acute Care (Long-Term Care) facility are required to obtain New York State Certification, issued by the NYSED, as a Dietitian-Nutritionist on or before November 28, 2021.
