The Crisis Care Coordinator is a highly responsible service position which will work in partnership with and be housed with the Llano County Sheriff’s Department Mental Health Unit. The Crisis Care Coordinator will perform crisis assessments in the field for adults and children as mental health calls or referrals are received. The delivery of services will be based on the needs of the community and individuals experiencing a psychiatric emergency. The Crisis Care Coordinator is responsible for assessing individuals in crisis situations, developing individualized crisis treatment plans including in-patient hospitalization referrals as needed, and providing crisis follow-up contacts. The Crisis Care Coordinator will also assess specific needs to be able to develop and implement Person Centered Recovery plans that support individuals with mental illness in learning skills to reach their specified service outcomes and to attain/maintain individual recovery. The Crisis Care Coordinator must provide timely and accurate documentation which supports diagnosis and outcome-based services. This position collaborates with intra- and inter-agency staff to improve services for the community. The Crisis Care Coordinator exercises extensive independent judgment and will help to maintain data, which will be used to plan and make projections for current and future services to meet outcomes. The Crisis Care Coordinator will work primarily with the LCSD’s Mental Health Unit and will adhere to their schedule, this position will also require some flexibility in scheduling due to the nature of responding to crisis calls.

  • Attends and participates in weekly/monthly staff meetings as required.
  • Maintains comprehensive clinical documentation that meets Dept. of State Health Services standards and Texas Health and Human Services Commission standards.
  • Properly utilizes Collaborative Documentation and enters within specified timelines.
  • Comply with HIPPA and PHI to ensure confidentiality of all client information.
  • Comply with all HCMHDDC and LCSD policies and procedures.
  • Attends work regularly and as scheduled.
  • Works cooperatively with others, Clinic Director, and Sheriff.
  • Communicates effectively and in a timely manner with team and individuals served.
  • Follow the agency workplace values and principles.
  • Maintain client / employee confidentiality and privacy according to standards.
  • Adhere to all Hill Country MHDD and LCSD policies and procedures.
  • Driving company or personal vehicle as required for service delivery and professional development.
  • Performs other duties as assigned by Clinic Director or Sheriff.
  • Job description may be changed to reflect job updates and requirements.
Qualifications

You’ll definitely need:

  • A bachelor’s degree from an accredited college or university in social, behavioral, or human services
  • Ability to be credentialed as a Qualified Mental Health Professional – Community Services (QMHP-CS)
  • Previous work experience in a team setting
  • 1-3 years of experience working in social services
  • Experience with crisis assessments and intervention as well as treatment planning and clinical documentation
  • Bilingual:  English/Spanish speaking – preferred
  • Live within 30 miles of clinic with reliable transportation - preferred
  • Maintain computer and software competency for program specific areas.
  • Maintain competency in Prevention and Management of Aggressive Behavior (PMAB)
  • Maintain certification in CPR AED
  • Responsible for staying current in mandated training and receiving additional training as appropriate.
  • A valid Texas State Driver’s License.
  • Current automobile insurance and the ability to travel when necessary.

  • Passion for connecting individuals and families with excellent care.
  • Sustain a calm demeanor when under pressure to handle sometimes difficult situations.
  • Provide exemplary customer service and care for your team.
  • Work autonomously to complete assignments.
  • Work cooperatively with others and be punctual and consistent in attendance because your people will rely on you!
