PURPOSE OF POSITION:
Coordinates the delivery of care services for individuals referred to the Hospital to Housing Program, including collaborating post discharge plans with hospitals, specialists, home health care and long-term care facilities, etc.
- Applies appropriate techniques and standards in program development and service delivery consistent with our mission, values and Catholic teachings.
- In collaboration with personnel (Patient Care Coordinator, Cae Managers and Administration), identify reported barriers to accessing benefits and housing and coordinates services to create a comprehensive and coordinated plan for addressing those barriers.
- Evaluates patients’ condition for eligibility into the H-to-H program. Evaluation includes but not limited to: Activities for Daily Living (ADL’s), health needs, and discharge plan requirements.
- Work cooperatively with other personnel (shelter, case managers and care providers) to help ensure patients receive comprehensive and coordinated care.
- Be knowledgeable of community resources appropriate to the needs of the patient.
- Assists program participant in completing benefit applications, attending follow up appointments and acquiring needed documentation as requested by the service provided agency.
- Effectively communicates to program participant; benefit programs services, requirements/agency procedures for obtaining and maintaining benefits.
- Maintain a high level of professional communications, customer service and follow through with residents, peers, referring agencies and supervisors.
- Monitoring H-to-H residents and communicating patient status including but not limited to vitals, observation of health/behaviors and logging of data in a clear and concise manner.
- Educate residents on how to communicate and advocate with family and/or community services agencies to ensure service needs are being met.
- In coordination with the housing navigator, identify and obtain essential documentation (ID’s, Birth Certificates and Social Security Cards) and resources to acquire permanent housing and benefits.
- Ensure ancillary services/equipment have been provided/rendered/delivered.
- Works with the housing navigation team to set realistic goals regarding permanent housing options.
- Coordinates transportation to follow up care as directed in the post discharge plan.
- Provides patients with guidance and education related to access of care and support for post program success.
- Participate in case management meetings/community meetings as needed.
- Implements program objectives and performance standards as directed by management.
- Implementation of budget as set by the manager and provide input into the process.
- Maintains accurate and timely program statistical data to meet reporting requirements. Enters data and generates reports.
- Preferred but not required; successful completion of an Associate’s Degree.
- Substitute education with equivalent combination of demonstrated on the job experience.
- CPR/AED/First Aid Certification and/or ability to obtain within 60 days of employment.

Salary: $20.00/hr, Full Time
