Clinical Case Manager

JOB SUMMARY: The Clinical Case Manager is a part time contractor position. This role operates under the supervision of the Program Coordinator and involves providing trauma-informed case management support, advocacy, and counseling to primary and secondary victims of crime. The Clinical Case Manager is responsible for ensuring that all consumers have timely and efficient access to community resources, legal support, and behavioral health services.
MAJOR DUTIES AND RESPONSIBILITIES:
Clinical Therapy (50%)
  • Provide a predetermined number of hours of counseling services to children, adolescents, and adults through telemedicine or in-person office visits.
  • Utilize evidence-based interventions, outcome measures, and clinical strategies to assist consumers in achieving positive outcomes.
  • Attend mandatory staff meetings, team meetings, and weekly clinical supervision meetings.
  • Adhere to organizational policies, procedures, and professional code of ethics.
  • Ensure timely entry of clinical documentation in accordance with organizational policy.
  • Collaborate with the front office, Program Coordinator, and Clinical Director to resolve any barriers to meeting weekly service-delivery requirements.

Case Management (50%)
  • Provide single point case management services, including needs assessments, trauma screenings, and follow-up for primary and secondary victims of crime.
  • Establish connections to housing support, emergency shelter, domestic violence arrangements, childcare, TPOs, behavioral health services, medical care, food programs, and travel assistance.
  • Work with clients to develop and maintain relevant and meaningful case management and safety plans.
  • Attend community meetings in Ward 7 and 8 to support community engagement and victim access to care, as assigned by the Community Engagement Specialist.
  • Collaborate with the Community Engagement Specialist on community events and social media engagement.
  • Work with the Victim of Crime Program team in managing social media posts, campaigns, and social media followers.
  • Participate in monthly and quarterly data and performance measure indicator reporting.

Skills and Competencies:
  • Demonstrate strong clinical skills and competency in working with victims of crimes, from assessments (including risk and safety) to planning and delivering care, and reviewing effective intervention methods to shape ongoing service provision.
  • Show strong skills in working with victims of crimes.
  • Effectively coordinate services, including natural supports.
  • Maintain sensitivity to religious, ethnic, and cultural differences in others, showing respect and empathy.
  • Must be able to provide accurate DSM diagnoses, use various treatment techniques for different diagnoses, and possess excellent knowledge of resources for victims of crimes.
  • Ability to manage change and support the agency's mission.
  • Demonstrate advanced skills in using Microsoft Products and Electronic Medical Records.

EDUCATION AND/OR EXPERIENCE:
  • Required: Master's Degree in Social Work
  • License: LGSW, LGPC
  • Preferred Experience: Two years of counseling using trauma-informed interventions, one year of case management experience, and one year of working with victims of crime.
