Job Title: Consumer Directed Services Coordinator
Status: Non-Exempt
Reports to: Director of Consumer Directed Services
Job Summary: The Consumer Directed Services (CDS) Coordinator reports to the Director or Assistant Director of CDS and is responsible for assisting persons with disabilities achieve goals for more independent living through the use of our consumer directed services program. Job activities include the following: case management services, payroll processing, documentation, and consumer education regarding the program.
Responsibilities:
Direct Service: Provide case management services to persons with disabilities; discuss social/human relations issues with consumers; assist consumers in assessing their needs and resources. At a minimum this must include a visit to each consumer in his or her home every 3 months and must include phone or email contact once a month.
Employer/Employee Paperwork: Process consumer and attendant paperwork; verify that it is complete and accurate in keeping with the approved plan of care, monitor for signs of falsified paperwork or misuse of the program.
Documentation and Document Management: Document goals and services provided; maintain clear and concise records including notes of all contacts; print case-notes monthly; file paperwork related to the program; compile activity reports as requested
Training: Provide training for consumers on how to utilize the CDS program; attend staff meetings and participate in staff training opportunities.
Other Duties: Perform other duties as assigned.
Qualifications:
The ideal candidate will hold a bachelor’s degree in a Human Services related field OR Have at least 2 years’ experience in case management services.
Excellent written and verbal communication skills, organizational aptitude, and the ability to manage multiple tasks with minimal supervision.
Strong Microsoft Office skills required.
Valid Missouri Driver’s License and reliable transportation.
Bilingual skills preferred but not required.
