CDR Case Manager/Care Specialist

South Community is hiring an AOD Case Manager. This is a full time position under the direct supervision of the SAS Program Manager. The CPSS functions as part of community based transdisciplinary team serving dual-diagnosis and high risk SUD adult populations. The general responsibility of the position is to provide case management interventions helping clients achieve their personal recovery goals.
The general responsibilities of this position are to provide community psychiatric support treatment intervention to clients 18 years of age or older who have multiple service needs. This includes developing mutual working treatment plans with clients and assisting clients in reaching treatment plan goals. The Adult Community Psychiatric Support Treatment Specialist will demonstrate time management skills, ability to work independently out in the community, and the ability to work in a larger team.
This candidate must have a bachelor’s degree or associate’s degree with at least three year of experience in the social work field. Access to an operational vehicle, automobile insurance ($100,000/$300,000 liability coverage) and a valid driver’s license is required. Must be able to pass a background check, motor vehicle check and drug test. Must maintain a current NPI number. Experience working with clients who have severe mental health problems strongly preferred.
