Community Outreach & Engagement Specialist

Department: Certified Community Behavioral Health Clinic (CCBHC)

Reports to: Director, CCBHC Services

Status: Full-Time

FLSA:Non-Exempt

Position Summary

The Outreach and Engagement Specialist provides mentorship, increases recovery capital, connects individuals and families to recovery support opportunities, and conducts baseline and follow-up data collection. The O/E Specialist is the individual’s liaison to referring agencies, crisis services, primary care, and other community supports as needed.

Required Qualifications (Knowledge, Skills, Abilities, Education, Training, and Licensure)

  • High School diploma required, Associate or Bachelor’s degree in a related field preferred OR an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job.
  • Lived experience related to substance use or mental health disorder recovery is highly desired.
  • If in recovery, we prefer you have at least two years of sobriety.
  • Knowledge of the local treatment community and referrals preferred.
  • Excellent customer service experience including active listening, problem-solving, and the ability to remain calm in emotional or stressful situations.
  • Communicate clearly and concisely, both verbally and in writing, and demonstrate a high level of listening skills.
  • Computer (PC) literate in general office functions such as word processing, email, and database software.
  • Commitment to HIPAA and ability to abide by standards of professional ethics and maintain confidentiality.
  • Current CPR and first aid certificate or willingness to be trained required within the first (90) ninety days of employment.
  • Must have a valid California driver’s license and proof of auto insurance that satisfies state requirements; along with a clean DMV record.
  • Knowledge of childhood/adult psychiatric disorders, trauma-informed care, substance use, and associated evidence-based practices. Cultural humility, trauma-informed care, and social determinants of health training are required.

 

Essential Responsibilities and Duties

  • Coordinate care with other services/agencies.
  • Conduct baseline National Outcomes Measures (NOMs) and follow-up data collection.
  • Complete timely documentation and electronic data reporting.
  • Practice mandated reporting/incident reports based on agency and state guidelines.
  • Provide monitoring of patients’ progress, and attendance and follow agency guidelines for contact.
  • Report to the CCBHC director and participate in regularly held case conference meetings.
  • This position will require travel between WellSpace Health locations as well as to other community locations as needed.

Physical Demands and Work Environment

The work environment is characteristic of a medical clinic environment. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is frequently required to sit; use hands to handle, or feel. The employee is also required to stand; walk; and reach with hands and arms. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus. The employee must also possess hearing and speech to communicate in person and over the phone. The noise level in the work environment is usually quiet.

 

The employee may be in contact with individuals and families in crisis who may be ill, using substances and/or not attentive to personal health and safety for themselves or their homes. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and the lack of personal care. The employee may also be exposed to bodily fluids (blood, urine) and hazardous chemicals.
