A DAY IN THE LIFE OF A COMMUNITY HEALTH WORKER
In this position, you will provide follow-up support services for at-risk older adults living independently. You will assist clients with application assistance, navigation of community resources, and provide and promote evidence-based health programs. You will work closely with our Return to Community team to assist the clients with their patient-centered support plan. The position includes in-person assistance in clients' homes and will involve direct client contact, requiring empathy and sensitivity.
This position is temporary through 9/2024.
QUALIFICATIONS
2 years experience working on Community Health projects, programs, or initiatives
- High School or GED Diploma. Preferred college education in health, human services, or related field
- Community Health Worker Certification is not required but preferred
- Reliable transportation: Must reside in MN or WI to be able to travel to meet clients

WORK SCHEDULE
This position is hybrid, working in the community, in the office, and remotely Monday - Friday, 8:00 AM - 4:30 PM
