We Are Hiring:Community Health Worker LPN - Sr Health Services Full Time
Schedule Details: Senior Health Community Health Worker will work closely with Senior Health Center staff providing support to adult patients, particularly those 65 and older, in their homes and in the clinic setting. You will interact with patients/families who are living with varying degrees of cognitive impairment.
Responsibilities: phone contact, scheduling appointments, home visits as well as clinic visits, various screenings, and educating patients/families so they have resources to address immediate care needs as well as for future planning.
Typical work hours: M-F 8-4:30

Job Summary
The Senior Health ServicesCommunity Health Worker (CHW) will be responsible for offering assistance and support to patients and their families with accessing and navigation of resources and services focused on promoting health and well-being. As a member of the Senior Health Services team, the CHW will work closely with providers to ensure patients and families have access to the tools and resources important to immediate care needs while also helping to address future level of care issues and a safe healthy living environment through an integrated approach to care management and community outreach. The CHW will outreach to patients and provide social support and serve as an advocate for individual and Community Health needs. The CHW will also track, verify and enter quality data
Responsibilities
The CHW will be a member of the Senior Health team and work collaboratively with all members of the team.
The CHW will assist the Senior Health Nurse Practitioners with pre-appointment screening for Seniors waiting for evaluation through the SHS department.
The CHW works collaboratively with the Senior Health and Behavior Health team members to schedule home visits when needed both pre-evaluation and for post-evaluation follow-up. Completing home safety assessment and caregiver burden scale.
The CHW will identify specific learning needs while reinforcing information shared during evaluation appointment.
The CHW will assist with monitoring and evaluating medication adherence in the home, identifying opportunity for education and/or assistance.
The CHW will assist with set-up and oversight of medication adherence using pill planners and other assistive tools to facilitate improved compliance.
The CHW will assist with completion of various screening tools such as the GAD-7 and PHQ-9, sharing details and observations with providers while documenting findings.
The CHW can monitor baseline vital signs such as : Height, weight, blood pressure, documenting and reporting observation data.
The CHW can assist with Advance Directives through retrieval of existing documents for scanning into EPIC. The CHW will be trained as an Advance Care Planning facilitator to facilitate completion of documents with appropriate individuals and ensuring completed documented are scanned into EPIC.
The CHW will be available to support the various data collections elements identified through the grant, as well as, any additional data tracking to support the program needs.
The CHW will be knowledgeable about the various community based resources available to support seniors and their families such as : ADRC, Alzheimer's Association, support groups, family care, etc. then assisting patients and families to accessing appropriate resources.
Assist patient by answering questions, providing program information and education about program services, guidelines, and policies in an accurate and courteous manner. Provide telephonic, small-group, and face-to-face interaction, information, and/or referral services to patients/families managing various chronic health conditions utilizing established protocols, procedures, and educational methodologies in an accurate and courteous manner. Encourage active adoption of habits that are conducive to a higher quality of life.
Utilize motivational interviewing techniques and communication skills to solicit change and/or increase customer desire to change behavior and adopt a healthier lifestyle as appropriate; document patient activities and meeting sessions in established format using EHR technology. Identify barriers to the patient's ability to stick to care plan and connect patient to appropriate resources and referrals to resolve them.
Work collaboratively within a team setting to achieve positive outcomes and meet department and company goals and objectives. Assist in the maintenance of community resource catalog, based on localized knowledge and other community services. Participate in home visits and community outreach.
What You Will Do:

What You Will Need:
  • High School Diploma
  • Licensed Practical Nurse (LPN) in State of Wisconsin
  • BLS through American Heart Association, or ability to obtain at time of hire.
  • Minimum of 2 years of experience
