About the job:
Community Health Worker advocates for the needs of their patients and helps them access services including but not limited to employment, housing, food programs or medical assistance. This position requires you to have a deep understanding of the challenges faced by those living in poverty, extensive knowledge of local resources designed for assistance, as well as compassionate communication skills.
What you’ll be doing:
- Works with patients with complex social issues to identify appropriate resources. Assist patients with navigating social services programs such as housing resources, food programs, legal services, transportation, etc., by helping them connect with community-based providers as applicable.
- Act as a patient advocate, linking and refer patients and their families with appropriate health care and social service/community resource networks needed.
- Work collaboratively with medical providers and other clinical personnel by completing follow-up with patients via phone calls and in-person visits.
- Establish trusting relationships with patients and their families.
- Works with agencies, providers and clinical staff to trouble shoot problems in communication regarding voucher and specialty care programs.
- Initiate appropriate forms for workers comp, disability, etc. as applicable and pass on the appropriate clinical providers to complete to ensure patients can receive their benefits in a timely manner.
- Provides an active voice in team meetings for the purpose of improving services to patients and maximizing patient benefits.
- Works with Population Health Manager to conduct outreach programs.
- Continuously expands knowledge and understanding of community services and resources; maintains frequent contact with community agencies serving vulnerable populations and participates in community meetings.
- Assist with clinical administration duties such as patient check out and scheduling follow-up appointments. May cover Patient Service Representative duties as required.
What you’ll need to be successful:
- Education
- Minimum - A minimum of a High School diploma or equivalent is required.
- Preferred - associate degree in health care, social services or related field or high school diploma or equivalent with at least two (2) years health care/ social service experience
- Basic computer skills and experience with Microsoft Office Suite
- Proficiency with electronic medical records
- Fluent in Spanish and English highly preferred.
- Ability to multi-task in a fast-paced environment.
- Effective communication skills.
- Commitment to patient and customer service.
- Ability to problem solve in individual and group settings.
