Description
The Community Health Worker II (CHWII) promotes comprehensive health and well-being within the community they reside in. CHWs are an integral member of patient-centered health teams. They walk along side neighborhood residents to achieve and maintain their ideal state of physical and mental health through prevention, education, and access to appropriate treatment. The CHWII can complete basic tasks, documentation, and problem solving without direct supervision due to their education or community experience. The role of the community health worker is complex and will vary in response to the needs of individual neighborhoods.
