We're hiring a Community Health Worker to help serve the Missouri Bootheel. Join our team today. Hours: Monday- Thursday 8 AM - 5:30 PM, Friday 8 AM-12 PM.
Job Summary: The Community Health Worker (CHW) will be responsible for helping patients and their families navigate and access community services, health care coverage, resources, and adopt healthy behaviors. CHW’s help bridge cultural and language barriers and improve health care outcomes. A CHW also builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy. Qualifications: Community Health Workers (CHW) are expected to possess a high school diploma and preferred to have a Community Health Worker certificate from an accredited university (one will be required after hire). CHWs should be knowledgeable, resourceful, have basic computer (Word, Excel) and medical terminology skills, and effective in people and communication skills. CHWs should be knowledgeable of their community and have experience working in a community based setting.
SEMO Health Network serves Scott, Mississippi, Stoddard, New Madrid, Pemiscot, and Dunklin counties in Southeast Missouri. Therefore, the position can be based out of the office closest to the candidates’ home.
