Community Health Worker

Position Purpose:

The Community Health Worker (CHW) primarily works in the community with a target patient population and their family, medical providers, primary care teams, and other agencies to improve the patient’s care and outcomes.

Coaches the patient and family in effective management of their chronic health conditions and self-care.

Establishes positive, supportive relationships and provides feedback.

Assists patients in utilizing resources they may be eligible for and identifying those not available through existing community resources.

Assists in reducing cultural and socio-economic barriers between a patient and community agencies or services.

The CHW continuously expands knowledge and understanding of community resources, services, and programs; human relations and the procedures used in dealing with the public as part of a service; volunteer resources and the practices associated with using volunteers, operations, functions.

Qualifications:

This position requires a high school diploma and relevant experience. Technical certificate or associates degree in Community Health work or a closely related field is preferred. Must be able to obtain Community Health Worker credential through Missouri Credentialing Board.

Must pass background check provided for the agency by the U.S. Department of Justice and the Family Care Safety Registry which includes the Missouri State Highway Patrol, Department of Social Services, Department of Mental Health and Department of Health and Senior Services. Must not be on any “do not hire” list with the Department of Mental Health or the Missouri Division of Family Services.
