JOB DESCRIPTION:
Life can be hard, and we think it's really important to help our community. We're looking for caring people to be Community Health Workers (CHWs) in and around Tennessee
As a CHW, you'll be a big help to patients in our community. You'll listen to their problems and help them during tough times. You'll work like a friendly neighbor, giving support for things like health, money, and community.
We need CHWs who can support patients now. You'll help them get the care and things they need, and you'll make sure their families have what they need too.
If you really want to help people in our community and you know how to listen and solve problems, we want you to apply.

Can handle tough situations and find good answers
Can talk nicely and get along with others

- As a Community Health Worker, you will support clients and connect them to resources as they work to achieve their personal goals and better their health. Imagine you have 10 clients. One will be discharged from the hospital tomorrow and needs transportation and home nursing services set up. Another client is 7 months pregnant and is currently unhoused because the shelters will not take her boyfriend in. You’re behind on your paperwork from last week. One of your coworkers is on vacation this week and one of her new clients is expecting to be contacted. You’ve been asked to help out. How would you organize your time to stay on top of your responsibilities?
