Job Description
General Responsibilities:
The Care Management Outreach Worker is responsible for providing administrative, clerical and patient engagement support to the Community Health Team (CHT). The Community Health Team is a team made up of Nurse Care Managers, Community Health Workers and Behavioral Health Clinicians. This is a community-based team who supports patients at Thundermist Health Center who are dealing with complex health issues.
Required Qualifications:
High School diploma or working in High School Internship program
Typing/Computer Skills/Telephone Outreach and Customer Service Skills
Ability to work well independently and to take direction from others
Highly organized and strong interpersonal skills
English, written and spoken language required.
Spanish, Portuguese, Laotian or other foreign language skills a plus.
Physical Effort/Environment:
While certain accommodation can be provided, this position normally works within the office setting and requires:
Manual dexterity is needed for keyboard work, writing and phone calls.
Ability to work in multiple, non-traditional settings and adapt to a changing work environment as needed.
May be required to transport and/or accompany CHT staff on home visits on occasion.
Work Schedule Demands:
As scheduled during hours of operation (including weekend and evening hours as assigned).
Communication Skills:
Strong oral and written communication skills.
Strong customer service and telephone skills, with patients and family members/care givers and community resource representatives.
Confidentiality of Information:
A high level of confidentiality. Full access to medical records. Must maintain all policies of confidentiality at all times and use discretion surrounding agency information and management issues.
Significant Job Functions:
Provide administrative, clerical and patient engagement support to the Community Health Team members.
Schedule patient appointments
Make appointment reminder calls
Schedule transportation for patients to relevant appointments
Identify required information in the Electronic Health Records (EHR)
Schedule meetings
Data entry and documentation in the EHR and other files
Filing
Copying
Faxing
Assisting with mailings
Assisting with special events
Assembling patient education materials into packets
Ability to prioritize workflow and handle multiple demands
Familiarity with office equipment and procedures
Other duties as assigned
