Description
The Public Health area of Health and Human Services is seeking a Community Health Specialist to work as a part of the Baby Tracks/IQIP team. The focus of this position will be community outreach, community education, partnership development and baby tracks interventions. Baby Tracks is an innovative immunization reminder program designed to help reduce the incidence of vaccine preventable disease by ensuring families with small children have information and access to immunizations by outreach, education and case management thus increasing population immunization coverage levels.This position will work to heighten community outreach and partnership development in diverse communities in Hennepin County, lead the work on educating and advancing health and racial equity on the Baby Tracks/IQIP team, and provide immunization reminders to parents of young children.

Effective December 31, 2023, there will be a general salary adjustment of 2.5% to the minimum and maximum of the range.

Location and hours:
This position is remote. Remote workers may be required to come on-site an average of 0 to 12 days a year for work meetings, trainings, community outreach or for any other reasons that their supervisor may deem necessary (may include additional days for onboarding and new employee training). Supervisors will provide as much advance notice as practicable. Work hours are Monday through Friday, between 8 a.m. to 4:30 p.m. Based on business needs of this position, hires must live in or relocate to Minnesota.
New employees who are hired into remote or hybrid positions between January 2, 2022 to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.

In this position, you will:
  • Be the Baby Tracks liaison that develops, strengthens and grows relationships with programs, both external and internal to Hennepin County, that serve families with young children such as WIC, C&TC, schools, daycares, religious institutions, and other community organizations to educate on the Baby Tracks program and the CDC childhood immunization schedule.
  • Plan, design, implement and evaluate childhood immunization community outreach, community education, and partnership development initiatives to increase childhood immunization rates in Hennepin County.
  • Organize and work with supervisor to assign staff to community outreach events designed to increase community knowledge about the importance of immunizations.
  • Collect data on community outreach events and results, develop quarterly reports.
  • Present to community partners on childhood immunizations, the CDC immunization schedule, and the Baby Tracks program.
  • Work with supervisor to develop community childhood immunization materials as needed.
  • Lead the work on educating and advancing health and racial equity in the Baby Tracks/IQIP team.
  • Assess and monitor Baby Track participant's immunization status, case manage when needed by encouraging parents in getting and keeping their children up to date with immunizations.
  • Educate parents about preventive healthcare and immunizations, provide referrals for low-cost clinics that provide vaccinations.
  • Prepare and maintain records, reports and parent outreach in Baby Tracks database.
  • Research immunization and client information in MIIC Program (Minnesota Immunization Information Connection)
  • Provide training to new staff as required.

Need to have:
  • Ability to communicate fluently in Somali in addition to English, both verbally and in writing.
  • One of the following:
    • Bachelor's degree or higher in public health, nursing, behavioral/social science, Somali language or an approved related field.
    • Four years or more of experience performing Community Health outreach.

    • Participating in multidisciplinary or community initiatives to improve Community Health.
