This position is for a Community Health Specialist (Community Health Worker) who supports the mission of “Saving Lives, Instilling Hope and Restoring Relationships” in an integrated team with patients receiving counseling, case management and medically assisted treatment. The Community Health Specialist (Community Health Worker) responsibilities include, but are not limited to the following:
- Coach patients in effective management of their potential health conditions and self-care.
- Assists patient in understanding care plans and instructions.
- Assist patients as a connector to obtain insurance in the marketplace and work with all uninsured patients obtain appropriate coverage.
- Motivates patients to be active and engaged participants in their health and overall wellbeing and overcome barriers to obtaining needed medical care and /or social services.
- Facilitates patient access to community resources, including locating housing, food, budgeting, clothing, prenatal classes, parenting, and providers to teach life skills, and relevant mental health services.
- Will assist with other clinic duties as it pertains to patient retention (engaging with patients in lobby, attending group sessions to market services, etc.).
- Work with Business Development Specialist and local leadership to continuously expands knowledge and understanding of community resources and services.
- Will attend community outreach events and Health Fairs as scheduled by Supervisor.
- Document all patient encounters in Spero Health’s electronic medical record in a timely manner.
- Stay informed of changes to health insurance and healthcare systems. Identifies gaps in healthcare services and develops program plans to meet those needs.
- Acts in compliance with contracts, laws, regulations, Spero Health policy, procedures, HIPAA policies and standards.
- Engage in continuing education to advance knowledge and understanding of the needs of Spero Health’s patient populations.
EDUCATION:
- High-school graduate or equivalent
EXPERIENCE:
- Prefer 1 year experience in clinical setting related to community outreach, conducting health promotion activities, assisting patients in navigating community resources.
LICENSURE/CERTIFICATION:
- Community Health Worker Certification (KY) required
