SCHEDULE:


Full-Time, Monday – Friday, 8:00 AM – 5:00 PM;

Rotating Saturdays (8a – 12p) / Weekend availability


PAY RANGE
: Salary: $45,000 - $50,000

WE ARE SEEKING A TALENTED PROFESSIONAL : COMMUNITY HEALTH NAVIGATOR

AccessHealth was selected to provide community navigation services as part of the United Way of Greater Houston’s Second Century Vision. AccessHealth currently has an operating navigation team who assist clients to access, apply for and obtain services and programs they need to meet personal, financial, educational, or social goals. The Community Health Navigator will be responsible for connecting patients and community members with local resources. The Community Health Navigator for AccessHealth is a frontline public health worker who serves as a liaison and link between AccessHealth clinic and health/social services.

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:

  • Represent AccessHealth and collaborative at meetings and events for networking, partnership building, collaboration, and referrals.
  • Review the social determinants and needs of patients.
  • Serve as a health coach for patients and community members.
  • Assist assigned population with accessing resources, including, but not limited to food, health insurance, housing, quality care, transportation, and health information.
  • Follow up with assigned group of patients to ensure access to care.
  • Link clients with appropriate resources and referrals to outside agencies.
  • Participate in department meetings, engage in health fairs, community meetings and presentations to increase community outreach.
  • Properly document activities performed, and assistance provided in the client’s chart in a timely manner.
  • Contact patients between visits to follow up on status.
  • Serve as a liaison between clinic and community resources / organizations.
  • Participate in job related training and development in new tasks and technologies.
  • Comply with all company policies, procedures, and regulatory requirements.
  • Other administrative duties as assigned.

EDUCATION / LICENSURE / CERTIFICATION QUALIFICATIONS:

  • Associates’/ Bachelor’s Degree
  • Bilingual in English & Spanish preferred.
  • Experienced working in areas of resiliency, disaster recovery, outreach, advocacy and/or social services.
  • Ability to work independently and as part of a team.
  • Exceptional communication, interpersonal, time management, and organizational skills
  • Case file management experience.
  • Intermediate computer skills in Microsoft Office Suite applications other job-related applications.
